New Patient Registrationregister_now

When you register with the Practice you will be asked to fill out the 'Application to Register with a General Medical Practice' form. This form requests information about your previous GP and your current and previous addresses. We also ask you to complete a basic questionnaire about your medical history and any medications you are currently taking.

We will also need-

Your medical card.

You should also bring proof of identity and evidence of your new address.

If you cannot find your medical card, don't worry as you will still be able to register with the Practice.

On completion of the form you will then be registered with the Practice.

When joining the Practice you are registering with The Practice and not with an individual GP. However, you may ask to see any doctor and the Practice shall endeavour to comply with your preference.

Upon completion of these forms, one of our receptionists will make you a registration medical appointment with the practice nurse. Please bring your urine sample to this appointment along with all recently prescribed medication.
