Patients please note that Patient Online Service is now Available.

Please contact Reception for a registration form.

For security you should provide ID to the Receptionist before a registration form can be given.

If you are currently registered for Online Services you should receive an email with details on how to change your login to the new service.

Should you require any further updates please contact the Surgery.

New Patient Registration

Our Practice covers Glasgow postcode area G44 4** & G44 5** and Lanarkshire postcode area G73 2** & G73 3** only.

To register as a patient at the Practice, you may call in to reception at your convenience. An application to register permanently with a general medical practice (GPR) form and a new patient medical history questionnaire. The GPR forms must be completed and signed for all family members.

We will also need 2 forms of Identification, one must have your current address on it.

When joining the Practice you are registering with The Practice and not with an individual GP. However, you may ask to see any doctor and the Practice shall endeavour to comply with your preference.

New Patients over 16 years

Upon completion of these forms, one of our receptionists will make you a registration medical appointment with the practice nurse. Please bring your urine sample to this appointment along with all recently prescribed medication.
