Online Service


Patients please note that Patient Online Service is now Available.


Please contact Reception for a registration form.


For security you should provide ID to the Receptionist before a registration form can be given.


If you are currently registered for Online Services you should receive an email with details on how to change your login to the new service.


Should you require any further updates please contact the Surgery.

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer, from reception or on the HMRC website.

Evidence that you are sick

Sickness CertificatesIf you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).

If this is the case you will need to make an appointment with your doctor to be issued with a fit note.
